Here we will introduce you to CDC diabetes prevention program. Health on Focus implements the proven CDC Diabetes prevention program.

 The CDC-led National Diabetes Prevention Program’s lifestyle change program can help people with pre diabetes prevent or delay type 2 diabetes and other serious health problems and improve their overall health. It’s scientifically proven, and it works.

More than 84 million US adults—that’s 1 in 3—have prediabetes, which means that blood sugar is higher than normal but not high enough yet to be diagnosed as diabetes. People with prediabetes are at high risk for type 2 diabetes (the most common type of diabetes), heart disease, and stroke.

According to the CDC , In the last 20 years, the number of adults diagnosed with diabetes has more than tripled as the US population has aged and become more overweight. Now more than 30 million Americans have diabetes, which increases their risk for a long list of serious health problems, including:

  • Heart attack

  • Stroke

  • Blindness

  • Kidney failure

  • Loss of toes, feet, or legs

People are more likely to have prediabetes and type 2 diabetes if they:

• Are 45 years of age or older;

• Are overweight;

• Have a family history of type 2 diabetes; • Are physically active fewer than three times per week; or

• Have been diagnosed with gestational diabetes during pregnancy or gave birth to a baby weighing more than 9 pounds.

PreventT2 is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). It is proven to prevent or delay the onset of type 2 diabetes. Research shows that modest behavior changes, such as making better food choices and increasing physical activity, reduced the risk of developing type 2 diabetes by 58 percent in people at high risk for developing this disease.

PreventT2 PROGRAM FEATURES :

Group sessions

Dedicated to You

learn how to eat healthy, add physical activity to your routine, manage stress, stay motivated, and solve problems that can get in the way of healthy changes.

Each group session will last about one hour, and you will be able to confidentially share your weight and weekly minutes of exercise with your coach. This program does require us to monitor your weight and activity level.

The program will run one year length 

3 meeting a month in first six months, then 2 meetings a month for next six months.

 Prediabetes means your blood glucose (sugar) is higher than normal, but not yet diabetes. Diabetes is a serious disease that can cause heart attack, stroke, blindness, kidney failure, or loss of feet or legs. Type 2 diabetes can be delayed or prevented in people with prediabetes through effective lifestyle programs. Take the first step. Find out your risk for prediabetes.

  • IF YOUR SCORE IS 3 TO 8 POINTS This means your risk is probably low for having pre diabetes now. Keep your risk low. If you’re overweight, lose weight. Be active most days, and don’t use tobacco. Eat low-fat meals with fruits, vegetables, and whole-grain foods. If you have high cholesterol or high blood pressure, talk to your health care provider about your risk for type 2 diabetes.


  • IF YOUR SCORE IS 9 OR MORE POINTS This means your risk is high for having pre diabetes now. Please make an appointment with your health care provider soon.


  • HOW CAN I GET TESTED FOR PRE DIABETES? Individual or group health insurance: See your health care provider. If you don’t have a provider, ask your insurance company about providers who take your insurance. Deductibles and copays may apply. Medicaid: See your health care provider. If you don’t have a provider, contact a state Medicaid office or contact your local health department.
